Together at Home is our service for people who are unable to attend our group-based delivery.
Volunteers will be matched with a person living with Dementia and their carer and given training so that they can visit once a week to do some fun, social, creative, music-based or gentle exercise activities.
If you would prefer to work 1:1 instead of in a group, please submit an Expression of Interest Form and someone will get back to you as soon as possible.
Here is a short video showing the importance of this service on members and volunteers:
Role Description
Together at Home volunteers need to be knowledgeable, confident, reliable people our members can depend on for a regular form of weekly contact, either over the phone or face-to-face at their home.
Volunteers on the Together at Home programme must complete a 1 day training course before they can be matched to a member, and they will then contact that person once per week. Volunteers must update Together Dementia Support after each contact.
⭕️ Attend a 1 day training course specific to Together at Home programme delivery
⭕️ Contact their member regularly, once per week, either over the phone or via home visit
⭕️ Complete relevant post-visit/contact feedback forms and return to manager
